The NHS is improving the way your health information is stored and managed through the introduction of the Summary Care Record.

The NHS Summary Care Record is being introduced across all GP practices in Nottinghamshire to help deliver better, safer care. Your Summary Care Record is an electronic record of important information about your health and will contain the following information: allergies, previous bad reactions to medicines and current medications

It will only be available to health-care staff providing your NHS care and having a Summary Care Record will help ensure the right people have the right information at the right time. Staff will only be permitted to access information, via strict security measures, if they are involved in your treatment. The Summary Care Record will be available anywhere in England and healthcare staff will ask for your permission before they use it.

NHS Nottinghamshire County, NHS Nottingham City and NHS Bassetlaw will be writing to GP patients aged 16 and over in Nottinghamshire during the next 2 years to inform them of the introduction of the Summary Care Record. This pack will contain a letter detailing the introduction of the Summary Care Record, a leaflet with more information about the Summary Care Record project and an order form to request the information in different formats and languages (with a freepost envelope). The letter also details what you can do if you wish not to have a Summary Care record created for you.

If you are happy to have a Summary Care Record you do not need to do anything as the changes will happen automatically.

If you do not want to have a Summary Care Record you can use the opt out form sent to you in your Summary Care Record pack or you can altDownload the opt out form [Adobe Acrobat PDF - 514.65 KB]. Please return all opt out forms either to your GP Practice or the Freepost address.
