Our vision is to ensure that high quality services are available that offer dignity, choice and support at the end of life.

One of our 12 strategic initiatives is to provide people with more choice at the end of their life, whilst improving their quality of care.

We have developed a new care pathway to provide the best possible support and treatment for everyone who is terminally ill.  This includes giving every patient a personal care plan in their last year of life.

Death and dying is issue which is often avoided and ignored, despite being the one common inevitability we all share.  So we are working hard to tackle the taboo around death and dying, and to encourage patients to talk about it with their families

More than 6,000 people die in the county every year and it is hoped that our initiative will start the discussion about one of life’s biggest taboos.

If you need information about your specific illness, it is best to ask your GP, keyworker or specialist nurse.

The leaflets and links on the below contain general information that you might find useful if you are a patient with supportive and palliative care needs, or are caring for someone with these needs.
