The Good Practice Guidance Toolkit for care homes

All care homes in Nottinghamshire have received the ‘Good Practice Guidance Toolkit’ which aids improvements in their residents’ quality of care.

The toolkit, from NHS Nottinghamshire County, contains posters and information leaflets which talk through the guidance step-by-step for care home staff.  It should also improve relationships between GP practices and the care homes they visit. 

Four different scenarios are explained for when:

• You have a new resident arriving.
• A resident needs routine healthcare.
• A resident may need urgent care.
• A resident has been discharged from hospital.
