Long Term Conditions

What’s a long term condition?

There are many long term conditions and you may have more than one which you live with and manage every day.

These are some of the most common ones: Diabetes, Heart Disease or a heart problem, Asthma, Stroke, Parkinson’s Disease, COPD, Arthritis, Back problems/pain, Cancer, Epilepsy, but there are many others including rare conditions such as Sickle Cell or Pernicious Anaemia, and mental health conditions such as Bipolar Disorder or Schizophrenia.

If your doctor, nurse or other health or social care professional has said that you have a long term condition then ask them how a care plan would help you or use the links to information on these pages.
